COLLECTION MANUAL
The Collection Manual details the sample collection procedure for AI Surveillance participants. It should be used by participants as a guide for collecting samples from birds for AI testing. It includes the appropriate sampling techniques, how to label collected samples, how to fill out the necessary sample data sheet, and where and how to ship samples.
Avian Influenza Surveillance Plan
This document is the fully detailed initiative for the USDA AZA Avian Influenza Surveillance Plan. The document provides an overview of the avian influenza risk, the role and responsibilities of participating organizations, and links for additional information.
Demographic Data Form
This form needs to be filled out by participants of the AI surveillance plan for every sampling event at their respective institution. It requires the participant to provide information on the animal being sampled from, including date of sample collection, species information, health status, and information on its living habitat. Upon filling out the form the information will be entered online, or mailed to the Zoo Animal Health Network.
Eastern Region Swab and Tissue Collection Data Sheet
This form is to be filled out by Eastern Region participants of the AI surveillance plan when swab or tissues samples are collected for AI testing. It requires the participant to provide sample specific information, such as animal ID and sample type. Upon completion, this form will be sent along with its respective samples to Cornell University laboratories.
Central Region Swab and Tissue Collection Data Sheet
This form is to be filled out by Central Region participants of the AI surveillance plan when swab or tissues samples are collected for AI testing. It requires the participant to provide sample specific information, such as animal ID and sample type. Upon completion, this form will be sent along with its respective samples to University of Minnesota laboratories.
Western Region Swab and Tissue Collection Data Sheet
This form is to be filled out by Western Region participants of the AI surveillance plan when swab or tissues samples are collected for AI testing. It requires the participant to provide sample specific information, such as animal ID and sample type. Upon completion, this form will be sent along with its respective samples to UC Davis laboratories.
Outbreak Management Plan
The Outbreak Management Plan provides a risk-based approach to preventing exposure to, and mitigating the spread of Avian Influenza, should it be detected.
Biomaterials Transfer Agreement
This is an agreement form between the AI surveillance participant collecting serum, and ZAHN at the Lincoln Park Zoo. The agreement explains ownership of samples sent to ZAHN and details the protections given to the submitter with regards to the extent of research any samples may be used for and confidentiality of test results.
Participation Checklist
This document is a list of necessary documents needed in order for an institution to participate in this surveillance program.

This document is a sample letter to State Veterinarians to endorse your institution's participation.

This form letter is a sample which may be customized for use in contacting your State Veterinarian.
Serum Collection Data Sheet
This form is to be filled out by participants of the Surveillance Plan for Avian Influenza when serum or plasma is collected for AI surveillance. It requires the participant to provide sample specific information, such as animal ID, sample type, and date collected. Upon completion, this form will be sent along with its respective samples to the Zoo Animal Health Network at the Lincoln Park Zoo.
